To apply for the Chalik & Chalik Cancer Survivors Scholarship (Scholarship), applicants must agree to the following terms and conditions.
To qualify for the Scholarship, applicants must be meet the eligibility criteria listed on the application page.
Applicants must submit their application and required forms using application form on the application page; applications submitted in any other manner will not be considered.
Applications must be submitted by the deadline indicated on the application page. Applications submitted after the deadline will not be considered. Chalik & Chalik Injury Lawyers is not responsible for technical problems that prevent submission of an application before the deadline.
Anyone who meets the following conditions is NOT eligible to apply:

Employees or children of employees of Chalik & Chalik Injury Lawyers

Past recipients of the Chalik & Chalik Cancer Survivors Scholarship

Applicants who are unable/unwilling to be interviewed by a representative of Chalik & Chalik Injury Lawyers if he/she were to be awarded the scholarship.

Applicants who submit the same essay for multiple scholarships
Scholarship Committee Will Decide Winner
A scholarship committee will consider all qualifying applications to select a winner. The scholarship committee will base its decision on applicants’ qualification based on eligibility criteria listed on the application page, applicants’ essays, and other merits of each applicant. The scholarship committee reserves the right to request applicants submit additional documents or information.
The scholarship committee will not discriminate against applicants based on their race, religion, national origin, gender, sexual orientation, disability, pregnancy, or membership in any other protected category.
The scholarship committee’s decision is final.
Privacy
Chalik & Chalik Injury Lawyers will not use applicant information for any purpose other than to consider the applicant’s qualification for the scholarship, nor will Chalik & Chalik Injury Lawyers sell, share, or distribute applicants’ personal or contact information.
Chalik & Chalik Injury Lawyers reserves the right to post the scholarship winner’s name and essay, in whole or in part, on its website, social media accounts, or in a press release.
